I'm assuming CFO is short for Cottage Food Operation? If so, your county Health Dept may have some specific guidelines about what needs to be included on the label in addition to following the FDA guidelines for ingredient labels.
I like the look of it with the details super-imposed over my logo. I did a color version, which was good, too, but decided I prefer the more subtle one. I just print these on Avery labels to make things simple. Stick 'em on brown paper bread bags, and fill them with bread.

The warning that wheat bread contains wheat reminds me of the peanut butter warning it might contain peanuts.
Yeah, probably not necessary, but my first thought was to put "Contains Bread" just to be funny. :)
I think that is a excellent label for your bread. Very well done.
I know it sounds funny to list some things, but remember we live in a time when people have to be told not to put a plastic bag over their heads!
